Use this form to record your PAINT SELECTIONS for each room to be painted. This form becomes a part of the permanent record of your job. Any changes to these decisions must be immediately transmitted to Shiloh Painting in order to avoid repainting and additional expense. Enter the Ceiling Color ID & Name from the paint can label. E.g., SW 2671 Navajo White (Sherwin Williams Navajo White)

Choose the sheen for Ceilings

Enter the Wall Color ID & Name

Choose the sheen for Walls

Enter the Trim Color & Name

Choose the sheen for Trim
